At Phoenix Diagnostic Imaging we are dedicated to providing the best possible patient experience.   We offer many outpatient imaging centers providing you with the convenience of multiple locations. 

How to Schedule an Appointment:
Patients may schedule an appointment directly, but must bring a completed physician referral form with them to the appointment. Physician’s office can also call to schedule appointments.

Please provide 24 hour notice if you need to change your appointment so that other patients may be accommodated.

We pride ourselves on staying on schedule, which in turn will keep you on schedule.  Please arrive 30 minutes before your scheduled appointment time to verify registration information and provide health information related to your exam.

We cannot stress the importance of comparing current studies to previous exams. Therefore, please be sure to bring the previous imaging studies you might have had, even if they were performed elsewhere.
